PATIENT INFORMATION
Salt Functional Medicine is a fee for service model. No insurance is billed for your visit or services, but a superbill can be provided for reimbursement purposes. It is recommended that the first visit be in person to establish care. Patient engagement and routine follow up is expected during the course of treatment. Once an established patient of the practice, you will have a guided plan of care and access to clinical staff on routine basis.
