PATIENT INFORMATION
SALT Functional Medicine does not bill or participate with insurance directly. A superbill can be provided upon request for you to submit to your insurance company for possible reimbursement, depending on your plan and benefits.
Our care model varies by location. Jacksonville Beach offers membership-based care, while Honolulu currently operates on a fee-for-service model.
Your provider will work with you to develop a personalized plan of care based on your individual health needs and goals.
